The Perry County Health Department is pleased to provide the residents of Perry County with a comprehensive summary of the community’s health status. Understanding the prevalence of chronic health conditions, barriers in access to care, and other health issues can help direct resources where they will have the highest return and impact. To that end, community agencies and organizations will begin using the data to develop and implement a community health improvement plan to meet the community health needs identified through this assessment.
